Each scenario carries a setting, a history, what the patient will and will not disclose, and the language it runs in. Programs assign scenarios to cohorts and decide who sees what.

Each learner gets a budget of minutes shared across scenarios and languages. Learners use fictional details only, confirm it before every encounter, and rate the simulation when it ends. Instructors see the ratings and the minutes used.
